WORKSHOPS & SEMINARS:

***Mental Health Evaluation is specialized assessments conducted by an independent, impartial licensed qualified Mental Health Professional. The Evaluation is a lengthy process and includes the ability to assess, diagnose, provide comprehensive reports, make recommendation(s) and sometimes testify in court. Mental health evaluations can be used in dependency court cases and for probation/parole purposes.

INSURANCE:
Services may be covered in full or in part by your health insurance or employee benefit plan.
Changing Tree Wellness Center will check your benefit information and a tentative co-payment will be discussed with you. Changing Tree Wellness Center takes care of all insurance billing. As long as your insurance company pays the claim, you will only be responsible for paying the co-payment. If, for any reason, the insurance company does not pay a claim, you will be responsible for the full fee.
Therapists are “in-network” with Aetna, Blue Cross/Blue Shield, MHN, United, and others.

Reduced fee services are available.
CANCELLATION POLICY:
If you do not show up for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, you will be required to pay a $25 missed session fee.
